Headdresses in the Lolita fashion are wide strips of fabric, usually decorated with lace and bows, that typically tie under the chin, but they can also be clipped on, tied at the back of the neck, or attached to a headband. The most common shape is a long rectangle, but a ruffly circle, worn tilted to one side, is also a common shape.

Headdresses are iconic of the Old School Lolita style, and are sometimes ironically referred to as "Maxi Pad Headdresses" because their shapes are sometimes similar to the feminine hygiene product.
